Child/Relational Developmental Home Visitor
POSITION: Home Visitor

REPORTS TO: Program Coordinator

FUNCTIONAL DEFINITION: This position requires the ability to form relationships with families and their children ages 0-3. This position is responsible for the establishment of partnerships with family members, program staff and external agencies to assess a family’s strengths, and needs. The home visitor provides opportunities for families to make informed decisions and create their own family goals and implements program philosophies to assist families in reaching those goals.

The candidate will be responsible for the implementation of the Individualized Family Partnerships Agreement (IFPA) and creating developmentally and age appropriate activities to model for the family that will promote positive outcomes for EHS enrolled Infants and Toddlers.

DUTIES INCLUDE:

Visiting families in their homes and establishing working relationships.
Collaborate with the site team to design and implement strategies that support compliance with the Early Head Start Performance Standards.
Promote optimal development of enrolled children across the five child development domains.
Provide appropriate parent education.
Provide home visits to informal child caregivers to promote relationships between parents, children and caregivers and to insure integration of the individual child development plan into the caregiving environment.
Lead infant/toddler, parent and other groups in socialization activities 2 times per month.
Complete child development assessment every 4-6 months.
Maintain a caseload of 10-12 families.
Develop partnerships with families to create Individualized Family Partnerships Agreements (IFPAs). Develop Transition Plans collaboratively with EHS multidisciplinary team.
Ensure appropriate assessments, brokering, advocacy, goal planning, case management, referral and support services to families.
Facilitate access to other agencies and services.
Enhance participant involvement in program governance.
Recruit and enroll families for the EHS program.
Maintain complete case records and implement Head Start Family Information System (HSFIS).
Attending all necessary program meetings.
Other duties as assigned, such as accompanying families to health care and other social service appointments, social service agencies and child care settings
Respecting confidentiality.





Requirements:

BS in Child Development or Early Childhood or degree in related field preferred with experience in infant/toddler development, family support, case management, or a minimum of 2 yrs. experience working in a family support program or EHS program with direct experience in infant and toddler goal planning, home visiting and relationship building.

Must have Act 33 and 34 Clearances.
Must have experience with residents in low-income neighborhoods and families with young children.
Must have a working knowledge of services/resources in community.
Must have the ability to work collaboratively.
Must possess organizational skills, problem solving skills and case recording skills.
Must have the ability to work flexible hours including evenings and weekends.
Must be at least 21 years of age or older.
